How does carpal tunnel surgery work?

Carpal tunnel syndrome is a common condition in which the median nerve is compressed at the wrist. This leads to pain, numbness and tingling in the fingers, particularly at night. The operation aims to reduce the pressure on the nerve.

A regional anaesthetic of the arm, a so-called brachial plexus block, is sufficient for the procedure. It is carried out in a bloodless field and with magnifying loupes. During the operation an incision is made on the inner side of the wrist and the ligament of connective tissue that crosses it (the flexor retinaculum) is divided in order to give the nerve more room.

Where appropriate, benign soft-tissue tumours or thickened tendon sheath tissue that may be pressing on the nerve are also removed. At the end of the procedure the incision is sutured and a dressing applied. The operation can be carried out as a day case.

Can I use my hand afterwards?

After the operation a dressing is applied that leaves the thumb and fingers free. The fingers can and should be moved. The hand should not be left hanging down.

What does aftercare involve?

On the first day after the operation the large dressing is changed for a small one. The sutures are removed after 10-12 days. Until they are removed you will need to wear a rubber glove when showering to keep the wound dry.

Do I need physiotherapy?

Only in rare cases is therapy needed, and then it is more likely to be occupational therapy, for instance to soften a hardened scar.

When can I work or do sport again?

Office work is possible again after just 1-2 days. The same applies to sporting activity. Walking and jogging are possible straight away. With strength training or mountain biking you should wait 2-3 weeks.
